Drive Forward Foundation Referral Form

Guidance Notes

Essential criteria for a Drive Forward referral is that the young person is in a position to realistically secure and sustain employment, education or training within 12 months of referral – so they are not currently in crisis, or experiencing intense issues (e.g. high level substance misuse, street sleeping, serious gang/criminal activity) that would prevent them from achieving this, with the support of DFF.

If a referral is felt to be inappropriate, Drive Forward will refer the client back to the originating agency, with a detailed explanation as to why the client cannot be engaged at this time.

Does the young person consent for you to share this information, and for ongoing information to be shared between the referral partner, Drive Forward, any funders and other stakeholders involved in their journey with DFF?

At Drive Forward Foundation, we are dedicated to supporting care-experienced young people on their journey from Care to Career. Our mission is to empower young people aged 16 to 26 to achieve sustainable employment, independence, and fulfilment through personalised support, training, and opportunities.

If you are a young person aged 16–26 and would like to work with Drive Forward to help you take the next step towards your career goals, please fill in the referral form below. We’ll be in touch to discuss how we can support you on your journey.

We also welcome referrals from professionals, including social workers, personal advisers, key workers, and other support staff who are working with care-experienced young people. If you are referring a young person to Drive Forward, please ensure the referral form is completed in full so that we can provide the most effective support possible.
